§ 5.28.110. Application.
A.
Applications for licenses shall be made to the chief of police in writing on a form provided for that purpose.

If the applicant is an individual (sole proprietorship), the application shall contain his name, residence address and telephone number. If the applicant is a partnership or other noncorporate business entity, the application shall contain the name and address of each partner, principal or member thereof. If the applicant is a corporation, the application shall contain the name and address of each principal officer and the registered agent thereof.

Each application shall also contain (i) the name of the film for which the permit is sought, (ii) the applicable fee to be paid as provided in this section, and (iii) such additional information as may be needed for the proper guidance of village officials in the evaluation of the application.
D.
At the time of making application for a permit, the applicant shall deliver to the chief of police the motion picture film for which a permit is sought, and a permit fee in the amount of three dollars for each one thousand lineal feet of film or fraction thereof.
